Instructions
Complete the application form and print a copy of your statement page for your records.
A minimum of a $500 non-refundable deposit per booth is required to hold booth space.
Upon completion of the form, you will have the option to pay immediately by credit card or request an invoice to pay via check or credit card by mail.
Your booth selection(s) becomes valid when approved and accepted by the Midwest Podiatry Conference.
Once approved, a confirmation with your booth location(s), balance due and a list of exhibitor requirements with due dates will be emailed.
