Instructions
  • Complete the application form and print a copy of your statement page for your records.
  • A minimum of a $500 non-refundable deposit per booth is required to hold booth space.
  • Upon completion of the form, you will have the option to pay immediately by credit card or request an invoice to pay via check or credit card by mail.
  • Your booth selection(s) becomes valid when approved and accepted by the Midwest Podiatry Conference.
  • Once approved, a confirmation with your booth location(s), balance due and a list of exhibitor requirements with due dates will be emailed.
